CYSA Referee Send Off Report Template - PDF
Referee report template to provide to referee crew in the instance a player is ejected from the game and a player or coaches card is collected by the referee.
Coaches should always have two stamped envelopes with them and a blank referee report at all games. The referee report is to be given to the center ref if
any red cards are issued for either team.
In the instance an MCFC player or coach is ejected, the player pass must be sent with the send off report to Top Dunlap, who is the D2 referee coordinator
along with a stamped envelope made out to Eric Johnsen to return the player cards once a penalty is assessed. Envelope for Eric should use the following

The completed referee report is to be entered online following instructions on the Referee page, and alternatively if it cannot be entered electronically, it
must be sent on paper to the district referee coordinator with the referee report completed within 24 hours of the game. If a player is ejected by a visiting
team, then the report and player card should both be sent by the referee to the D2 coordinator at the address below. The envelope should be addressed as

manual. NorCal is the playing league for US Club Soccer teams playing in Northern California.
US Club Soccer Referee Send Off Report (Word format)
The Center Referee that had issued a Send/Off (Coach or Player) should fill out the MS Word version of the NorCal Referee Send-Off Report and E-mail
that directly to me albertchu1@Comcast.net. By doing this we save a lot of time in determining who the Send-Off Player/Coach is and what their game
suspension is going to be. We need to do this quickly so the suspension information will be available to the affected teams and coaches ASAP.
Coach's or Player's Pass (that were Sent-Off) should be returned to the team after the completion of the game. All Send-Offs/Ejections and Suspensions
will be tracked on the NorCal website and that other teams in the same division/group will be notified of the infraction and the pending suspension.
